Childcare Reimbursement Form

Parkridge Church believes participation in a small group is critical to the life and development of every member. For some families, childcare is a necessity for participating in a group. Through partial reimbursement, our church wants to help by lessening the burden of childcare costs.

Please submit one request that covers all meetings held during a calendar month. Requests must be submitted by the 15th of the following month. Reimbursement checks will be available by the 25th of the month and picked up at ParkridgeKids check-in desk.

MEETING WEEK ONE

The cost per childcare worker is $20/hr. per group meeting; not to exceed $40. The total cost will be divided evenly amongst each family that brings any number of children.
